3 Put in the date of completing this statement.

4 Put in date when worker commenced employment. It is also necessary to state whether or not any previous employment is counted as continuous with the current employment and if so when this earlier employment began. This continuity would occur where the employee had been employed by a previous employer on the same holding, or by the same employer in a different job.

5 Job titles have to be stated, i.e. Dairyman, Shepherd, General Agricultural Worker, etc. This is not a job description, but just the title.

6 In the first part put in the worker's classification under the Wages Structure, i.e. "an ordinary worker" or "a craftsman" or "Appointment Grade II" or "Appointment Grade I". In the second part, if the worker is paid the minimum rate for his classification, put in here "the appropriate Agricultural Wages Order". If the worker is paid more than the minimum rates, or less in the case of a permit worker, put in "the Wages Register", and show details of the wage paid in the Wages Register kept in the farm office.
7 State here the practice on the farm. The N.F. U. recommends that this should read: "You are paid weekly and on Fridays".

8 Set out in the Register the hours required to be worked including regular overtime.

9 If the worker is paid his normal wage when on holiday put in "the Wages Register" or "the Agricultural Wages Order" as appropriate. If he is paid the minimum wage set by the Order and this is less than his normal wage, put in "the Agricultural Wages Order".

10 The statement assumes that the statutory sick pay scheme set by the Agricultural Wages Order applies. Where, however, different arrangements are made these should be stated instead. (Different arrangements cannot be less favourable than the statutory ones.)

11 State here if the worker is covered by a Pension Scheme, and where the details are available for reference. If there is no Pension Scheme on the farm, say so.

12 The employee's notice entitlement is that required by the Contracts of Employment Act as amended. While the legislation only requires an employee to give one week's notice there is nothing to prevent a worker being required under his contract to give longer notice. He can, for example, be required to give the same notice as he is entitled to receive.

13 It is a legal requirement that there shall be a procedure for dealing with grievances. In most cases application would be made direct to the employer and the form should state "your employer". Alternatively it. might be "the manager" or "the foreman".

14 If there are any disciplinary rules applicable to the employee these must either be specified in the statement or reference must be made to another document in which they are set out. It is also necessary to set out, either in the statement or in another document to which reference is made, the person to whom the employee can apply to if he is dissatisfied with any disciplinary decision affecting him, the way the application should be made and any further steps that can be taken. These provisions do not apply to rules, etc., relating to health and safety at work.

15 If a house, board, lodging or other perquisites are provided, either free or at a charge as part of the contract, this should either be shown here on the statement, or reference made to the fact that details are entered in the Wages Register.

16 State where. It will usually be "the farm office". Note that no particular form of Wages Register is prescribed. It can be any book or other form of record which is available for inspection by the employee.

12 LENGTH OF NOTICE. You are entitled to receive 1 week's notice during the first
2 years of your employment. After that your notice will be 1 week for each complete
year of continuous employment up to a maximum of 12 weeks. You are required to
give two week(s) notice.
13 GRIEVANCES. Ip. the case of any grievance relating to your employment, a written
application stating the nature of the grievance should be made to your employer
The matter will be discussed with you and receive consideration, and you will be
notified of the outcome.
